Utah Tennis Hall of Fame Nomination Form
PURPOSE
To honor men and women who have made outstanding contributions to tennis in the State of Utah.
IDENTIFICATION OF CANDIDATES
Any group or person may submit a name for consideration to the UTAH TENNIS HALL OF FAME nominating committee.
Candidates will be nominated and elected on the basis of judgment gained from general public perception of the candidate and the accompanying informational questionnaire, plus any other written information provided by any interested group or person. The candidate will be judged on: (1) Tennis playing achievements. (2) Contributions of service to organized tennis. (3) Considerations of tenure, maturity and residence will be factors. Primary emphasis will be on tennis related accomplishments and service. Evaluation will be based on local, state, intermountain, sectional and national tennis accomplishments.
METHOD OF SELECTION
The UTAH TENNIS HALL OF FAME committee will review all applicants for nomination. The names of the official nominees will be placed on an official ballot and distributed to the electors, which will consist of the committee members.
A candidate to be elected must have 50% of the votes cast. It is entirely possible that years may pass without anyone elected. The President, Vice-President, and the Secretary of the UTAH TENNIS HALL OF FAME committee will count the votes. Ties will be broken by a vote of these three people.
TIME OF SELECTION
For the purpose of identifying the nominees for this award, the UTAH TENNIS HALL OF FAME committee will submit its nominations for election annually on a date selected by the committee. The induction ceremony will follow at the time and place deemed appropriate by the committee.
THE PHYSICAL FORM OF THE AWARD
A suitable award will be created. Accompanying this award should be a letter outlining the inductee's accomplishments and an expression of appreciation signed by the current UTAH TENNIS HALL OF FAME committee and the President of the Utah Tennis Association.
DISPLAY
A picture or caricature and brief summary of achievements will be placed on display at a suitable site or sites, in sequence of the award.
